Team Check In will be held at the following 2 times at the Field House at BB&T Soccer Park:
* Friday, January 25th from 4:00pm to 9:00pm
* Sarurday, January 26th from 7:00am to Noon
Please see info below for the necessary paperwork to bring to the Team Check-In.
This is a NON-RESTRICTED tournament which means that USYS and US Club sanctioned may participate. If your affiliation is with another organization, please contact the Tournament Director.
USYS TEAMS will be required to submit the following paperwork for the tournament:
One Official Match Roster (TCYSA will retain) + 3 photocopies - copies will not be made on site so please make sure you have all copies required
Laminated player cards/passes - one for each player and team official
Current Medical Release - one for each player
Permission/Notification to Travel: ALL USYS teams need Permission/Notification to Travel - this applies to team OUTSIDE of NC. If your state uses E-travel or other online PTT, a copy of this approval must be submitted at team check-in and retained by TCYSA.
1: Submit your state organization’s Guest Player Form + 3 photocopies. Note: If our guest player is from another state, a US Youth Soccer Interstate Permission Form is used in lieu of the other guest form.
2. Laminated 2012-2013 player pass
3. Current Medical Release form
US CLUB TEAMS will be required to submit the following paperwork for the tournament:
2012-2013 US Club team roster + 3 photocopies
US Club Passes, laminated - one for each player and team official
US Club Current Medical Release - one for each player
Guest Players: If player is not from your club and requires a US Club Player Loaner Form, this form must be submitted along with 3 photocopies. A laminated US Club pass and US Club Medical Waiver for the guest player must be presented at check-in.
NOTE: TEAMS MAY NOT MIX USYS AND US CLUB PLAYER PASSES.
TEAMS MUST BE ALL USYS, OR ALL US CLUB.
