| When completing the medical waiver
form for tryouts: |
Do not enter a team name or jersey number.
Player's name must be entered as it appears on the birth
certificate. No nicknames. A middle initial is
required unless player does not have one.
Parent must sign in presence of a notary.
If using the triplicate form, please ask the notary to stamp
all three pages. If using the single web version,
please complete and have notarized, and then make 2 additional
copies. All three copies must be submitted at tryout
check-in.
